The third step is measuring engagement, not just oversight. The question isn't "was a human in the loop?" It's "was the human paying attention?" A human who rubber-stamps every AI decision is not oversight. They're a compliance fiction — a warm body whose presence satisfies a regulation without providing the safety the regulation intended.

The autonomy gradient isn't a design choice you make once. It's a condition you monitor continuously. Systems drift toward more autonomy because autonomy is efficient and human oversight is expensive.
